📜 School Registration Agreement — September 7, 2026

YOUR PROGRAM AND ENROLLMENT
This agreement is for the named student and the school program selected in this form: Little Apollos Music & Play at $99/month or Piano Teams at $119/month. Lessons take place at the school named in this form and are taught by Ashton of Apollo Music Academy. Apollo supplies the instruments and materials used in these school classes; existing written school/family material promises remain honored.
Before payment, Apollo will provide your written enrollment summary with the start date, lesson dates and times, class duration, purchased live-class allocation, school breaks, any separate registration charge or optional purchase, and the amount due. Any tuition discount must be confirmed in writing. Your submission records agreement to these service terms; it does not accept an unseen schedule or undisclosed fee. Enrollment remains provisional until you and Apollo confirm the actual summary and the applicable payment conditions are met. No extra charge or annual-calendar class denominator will be imposed without prior disclosure and agreement.
PAYMENT THROUGH SQUARE
Apollo will issue the applicable Square invoice after reviewing registration and confirming the enrollment details. Square handles payment; this registration does not collect card details or authorize automatic charges, a card on file, or recurring payments. No second Apollo service contract is required solely to pay through Square. Any separate payment authorization must describe its own terms.
For ordinary monthly tuition, invoices are issued on the 28th, with a grace period through the 1st. One $25 late fee may apply on the 2nd if that invoice is unpaid. No daily fees accrue and the same invoice will not receive a second late fee in a later cycle. If tuition remains unpaid by the 15th, participation pauses on the 16th while unpaid. Contact Apollo promptly if payment is difficult. These monthly rules do not replace the separate terms of a fixed Fall course.
ATTENDANCE, BREAKS AND WITHDRAWAL
The confirmed school calendar identifies purchased class dates and planned breaks. If your child misses a group class, Apollo provides that week's practice and recap materials. A student absence does not reduce the agreed tuition or automatically include a private makeup.
Send withdrawal requests in writing to Contact@ApolloMusicAcademy.Online. Before you confirm enrollment, your written summary must state whether enrollment is ongoing monthly or for a fixed term, any initial commitment, the applicable notice deadline and when withdrawal takes effect. This provisional registration does not create an undisclosed minimum term, notice period or future tuition obligation. You may decline the proposed summary before confirming it without becoming responsible for tuition. After confirmation, the disclosed withdrawal terms apply. Student withdrawal is separate from Apollo's obligation to remedy live instruction Apollo cannot provide. Applicable legal rights remain available.
IF APOLLO CANNOT PROVIDE YOUR PROGRAM
If Apollo cannot launch the purchased program on the confirmed schedule, you may choose a suitable replacement offered by Apollo or a refund of tuition paid for the undelivered program. Apollo cancels unpaid tuition for that undelivered program and reconciles prior credits/refunds. A replacement or account credit requires your agreement. Apollo initiates the resulting refund within seven business days after confirming your choice and the amount due; payment-provider processing may take longer.
If Apollo cancels purchased live instruction, Apollo will arrange a suitable live makeup within 30 days. If that live instruction cannot be provided, Apollo will reconcile and refund the tuition allocated to the undelivered class. A recording, worksheet or practice assignment alone does not replace the purchased live instruction. Apollo initiates a resulting refund within seven business days after the undelivered amount is established.
For monthly enrollment, the written summary must state the purchased-live-class allocation before payment. For a fixed course, a class is valued at net agreed course tuition after tuition discounts divided by purchased live classes. Separate trials, registration and optional services are excluded from this tuition calculation. Apollo reconciles delivered instruction, payments and prior credits/refunds, reduces or cancels the applicable unpaid balance first, and refunds any excess tuition payment once without a duplicate credit. Applicable law controls if another remedy is required.
PARTICIPATION, COMMUNICATION AND PRIVACY
Music, movement and instrument activities require appropriate supervision and care. Please discuss participation or access needs privately with Ashton; no medical diagnosis is required in this general registration form. Follow the school's agreed arrival, handover and pickup arrangements. This agreement does not waive liability for negligence or require a blanket statement that a child has no disability or support needs.
Your contact information supports registration, scheduling, lesson updates and billing. A preferred contact method is not promotional-marketing consent. Identifiable publicity is optional and separate. Registration and payment do not authorize lesson recording or audio-to-text transcription. Before capture, Apollo will explain the specific use, retained outputs, access, storage and retention and obtain applicable separate permission. Without that permission, lessons continue with teacher-written notes at no extra charge.
